

Insurance Coverage
Depending on your insurance plan, you may be able to get Theraworx Relief at a reduced cost. Check with your insurance provider, pharmacist or doctor to find out if any of these benefits apply to you:
- Health savings account (HSA), flexible spending account (FSA), or health reimbursement account (HRA). You can use your HSA, FSA or HRA to pay for Theraworx Relief in the pharmacy or online. (See your plan rules for eligibility and how to pay or be reimbursed.) There is no need for a prescription to use your healthcare spending accounts for over-the-counter (OTC) products.
- Medicaid OTC prepaid benefits debit card. If you have this card, you may have a quarterly allowance to buy OTC products online or in pharmacies. This benefit does vary by state and by plan, so be sure to check your benefits coverage. You do not need a prescription to use your Medicaid OTC card.
- Reimbursement with a prescription. If you have visited your doctor and received a diagnosis related to muscle cramping or joint discomfort, you may be eligible for partial or full reimbursement from your insurance company as long as you have a prescription. Ask your doctor to write a prescription for either Theraworx Relief for Muscle Cramps and Spasms or Theraworx Relief for Joint Discomfort and Inflammation if you have received a diagnosis for any of the conditions listed in the section below.
Conditions That May Qualify for Reimbursement
Show your doctor the following list, or print this page and take it to your next appointment. If your doctor says you have one of the following conditions, ask him or her to write you a prescription for Theraworx Relief and to include the condition and code on the prescription (these codes are in parentheses next to the conditions below—they help the insurance company determine coverage).
Conditions Related to Muscle Cramps and Spasms
- Restless legs syndrome (ICD-10-CM G25.81 or ICD-9-CM 333.94)
- Sleep-related movement disorders (ICD-10-CM G47.6)
- Sleep-related leg cramps (ICD-10-CM G47.62 or ICD-9-CM 327.52)
- Abnormal involuntary movements/cramp and spasm (ICD-10-CM R25.2 or ICD-9-CM 729.82 - cramp of limb or ICD-9-CM 781.0 - abnormal involuntary movements)
- Pain in unspecified lower leg (ICD-10-CM M79.669 or ICD-9-CM 729.5 - calf pain, lower leg pain)
- Other muscle spasms (ICD-10-CM M62.838 or ICD-9-CM 728.85 - spasm of muscle)
